Of good moral character and has not been convicted of an offense involving moral turpitude
Holds at least a master's degree in psychology conferred by a university, college or school in the Philippines or abroad recognized / accredited by the CHED and has obtained sufficient credits for the subjects covered in the examination: Counseling and Psychotherapy, Advance Theories of Personality, Advance Abnormal Psychology, Psychological Assessment
Has undergone a minimum of two hundred (200) hours of supervised practicum / internship / clinical experience relayed to services enumerated in paragraph (b) of Section 3 of R.A. No. 10029 and under auspices of a license psychologist or other licensed mental health professional

Original and photo copies of Transcript of Records with Special Order, Date of Graduation, seal of the school, with scanned picture and remarks " For Board examination Purposes Only" (Res. 2004-200, s'04) (Ched recognition / permit to operate for graduates of new schools / programs)
Original and photo copies of Birth Certificate (Philippine Statistics Office is formerly known as NSO)
Four (4) passport size colored pictures with complete name printed
Metered Documentary Stamps
Community Tax Certificate / Cedula
Examination Fee: Complete (900.00 pesos)
Marriage Certificate (if any)
ADDITIONAL REQUIREMENTS:
Three (3) certificates of good moral character, preferably from school, employer, church & barangay captain duly signed by the issuing authority and duly notarized under oath.
